Advanced Practice Provider - Boulder Heart Boulder Community Health, Boulder, CO 80303

$52.48- $78.72 an hour - Full-Time, Exempt (Salaried) | Schedule: Monday, Wednesday–Thursday

Boulder Community Health is seeking an Advanced Practice Provider (NP or PA) to join our Electrophysiology and Cardiology team at Boulder Heart. This role provides comprehensive cardiovascular care across both outpatient and hospital settings, partnering closely with cardiologists and electrophysiologists to evaluate, diagnose, treat, and manage patients with cardiac and electrophysiology conditions. The APP will play a key role in supporting continuity of care, facilitating transitions between inpatient and outpatient settings, providing patient education, and delivering high-quality, patient-centered care throughout the cardiovascular care continuum.

Qualifications •Current Colorado Physician Assistant or Nurse Practitioner License. •Must be credentialed by the Boulder Community Hospital Medical Staff Office prior to hire date. •BLS certification required by the end of conditional period. •ACLS certification required within 6 months of hire. •Experience in Cardiology multi-specialty clinic preferred

Responsibilities •Conduct comprehensive cardiovascular assessments, including obtaining medical histories, performing physical examinations, reviewing records, and developing diagnostic and treatment plans. •Order, interpret, and integrate laboratory, diagnostic, and imaging studies to evaluate and manage cardiac conditions. •Diagnose, treat, and manage patients across the continuum of cardiovascular care, including medication management and ongoing monitoring between physician visits. •Collaborate with cardiologists, electrophysiologists, and interdisciplinary care teams to coordinate patient care and facilitate transitions between inpatient and outpatient settings. •Provide patient and family education regarding diagnoses, medications, treatment plans, disease prevention, and self-management strategies. •Triage patient calls, provide clinical consultation, and respond to urgent or emergent patient care needs as appropriate. •Maintain accurate, timely documentation, including clinical notes, orders, billing, and care coordination activities. •Participate in quality improvement initiatives, departmental meetings, professional development activities, and continuing education. •Maintain compliance with all regulatory, accreditation, safety, confidentiality, and infection prevention standards.
