Full job description
Liberty Home Care is looking for a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor! This Part Time opportunity offers great benefits and a consistent M-F 9a-2p - 25 hour per week schedule.
Schedule: 9am - 2pm Monday - Friday (25 hours per week)
Service Area: Niagara County
Duties & Responsibilities:

Provide assessment of newly referred patients and assess health needs in home care setting

Work with patient to create a patient centered plan of care

Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family
-
Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services

Provide supervision and training of PCA/HHA staff
Qualifications:
-
NYS Licensed Registered Professional Nurse
-
Minimum of Associate's Degree required
-
One year Nursing experience required
-
Ability to travel throughout Niagara County for home based visits

Home care experience a plus
