What to expect.
-
In collaboration with the patients' interdisciplinary treatment team, performs a full range of aftercare and discharge planning services geared to mentally ill patient population.

Maintains comprehensive knowledge base of agencies, organizations, and resources within the referral community for discharge planning and placement.

Communicates with patients based on an understanding of the developmental needs of the population.

Develops and assists in the implementation of discharge and aftercare plans; works with patients and families to facilitate the community transition and assure appropriate match to patients' needs.

Pay for this position is determined on a number of factors, including but not limited to, years and level of related experience.
What we need from you.
-
Bachelor's degree in a related mental health care field, such as Social Work, Nursing, Psychology, or a Rehabilitation Services specialty.

For internal candidates, high school education plus an additional 5 years of relevant experience (2 years of work must be within Sheppard Pratt) may be considered a substitute for a bachelor’s degree.

Knowledge of psychiatric terminology, discharge and aftercare planning methods, and community referral resources - acquired through six to twelve months of relevant field or work experience in a psychiatric setting.
