We are currently seeking a Clinical Supervisor to join our Specialized Recovery Services Program. This is a meaningful opportunity for professionals who are passionate about helping others live healthier, more independent lives. As a Clinical Supervisor, you’ll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. You’ll be part of a mission-driven team that values your expertise, supports your growth, and empowers you to make a real difference in your community. Minimum Qualifications

  • Must be either licensed RNs, licensed social workers or licensed independent social workers (LSW, LISW) with at least five (5) paid years of clinical experience in home-and community based services and a minimum of one (1) year of supervisory experience.

  • Certified Case Manager (CCM) required or obtained within one year of being in the position.

  • Experience in completing assessments, service plan development and monitoring.

  • Effective organizational and communication skills.

  • Experience providing care management/coordination preferred.

Key Responsibilities

  • Adheres to the CareStar Rule in performance of job responsibilities.

  • Understands and complies with CareStar Policies and Procedures.

  • Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.

  • Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

  • Directly supervises, manages and oversees staff, including hiring, firing and providing performance evaluations, including and individual development goals and objectives.

  • Provides direct clinical services to consumers as necessary to assist the clinical team.

  • Educates, counsels, evaluates, and manages performance of the clinical team.

  • Maintains/updates knowledge of Medicare, Medicaid, insurance and program specific administered waivers.

  • Preforms other projects and duties as assigned.
