Description
Crouse Community Center is seeking a compassionate and skilledPer Diem Speech Therapistto join our rehabilitation team. This flexible role is perfect for an ST who enjoys meaningful clinical work while supporting residents in a warm, collaborative long-term care environment.
As a Per Diem Speech Therapist, you will provide high-quality evaluation and treatment services to residents with communication, cognitive-linguistic, and swallowing needs. You will work closely with nursing, medical staff, and interdisciplinary team members to ensure individualized, person-centered care.
What You’ll Do

Provide ST evaluations and treatmentsfor communication disorders, cognitive-linguistic needs, and dysphagia.

Develop individualized treatment plansbased on resident assessments and clinical findings.

Document progress and assessmentsin accordance with regulatory and facility standards.

Collaborate with interdisciplinary teamsincluding nursing, medical staff, PT/OT, and social work.

Participate in MDS and care plan processesthrough timely communication and quarterly reviews.

Ensure safe therapy practicesincluding adherence to infection control and PPE protocols.

Maintain a professional, supportive environmentfor residents and families.

Requirements
What We’re Looking For
-
CurrentNYS Speech-Language Pathology licensein good standing.
-
Master’s Degree in Speech-Language Pathology from an accredited program.
-
Experience in long-term care or a similar healthcare setting preferred.

Strong communication, clinical reasoning, and documentation skills.

Ability to work independently and collaboratively within a multidisciplinary team.

Professional, positive, and resident-focused demeanor.
Salary Description
$40 - $60 per hour
