Transportation Coordinator (PT)

Location: Hempstead NY | Salary: $20-22/Hour | Schedule: Part-Time, 28 hours per week

The Opportunity

Make a difference in the lives of people and your community by helping them access services. The Transportation Specialist will be responsible for assisting and coordinating transportation for individuals to Family & Children's Association's Certified Community Behavioral Health Center for treatment services. The Transportation Specialist will provide direct services to clients by using a company van to transport individuals to the centers after being discharged from the hospital. The position includes helping oversee and support the agency food pantries.

Responsibilities

  • Coordinate transportation for clients attending treatment services

  • Schedule rides using online transportation systems

  • Transport clients safely using a company van when needed

  • Assist transporting hospital-discharged individuals to FCA's outpatient treatment centers

  • Monitor and facilitate the purchasing and distribution of OMNY cards.

  • Communicate effectively using a person-centered and strength-based approach with clients, treatment teams, and service providers

  • Enter client notes and transportation data into agency systems

  • Help maintain and support agency food pantries

  • Ensure compliance with federal, state, and agency regulations while maintaining client confidentiality and adhering to HIPAA policies and patient rights procedures.

  • Performs other duties as assigned.

Qualifications

  • High school diploma or equivalent required.

  • Must have a valid NYS Driver’s License for at least two years and have completed a defensive driving course within the past 3 years or complete the online course within 30 days of hire.

  • Preferred bilingual – Spanish and English speaking.
