Responsibilities
Serves as a case manager for the Holmen Boys and Girls Club assisting youth who struggle with behavioral and mental health challenges. Responsibilities include working with a multi-disciplinary team to create a safe and nourishing environment where kids can grow and develop skills needed to be successful in school and in the community. The case manager will facilitate groups, develop treatment plans with identified high risk youth and work individually with some youth to enhance coping and healthy living skills. This position will work collaboratively with the Boys and Girls Club staff and also work closely with area schools and community partners.

Qualifications

At least 1 year working with individuals with mental health and/or substance abuse issues.

Bachelor's degree in social work or in human services related field preferred.

Master's degree preferred.

Broad knowledge of the mentally ill population.

Effective communication and good listening skills.

Demonstrated ability to maintain confidentiality.

Excellent documentation skills.

Ability to work in a cooperative and team-like fashion.

$24.55 - $35.65 / hour. Education, experience and tenure may be considered along with internal equity when job offers are extended.
