Licensed Practical Nurse LPN

As our Licensed Practical Nurse (LPN), you will provide compassionate, hands-on care while coordinating with residents’ primary care teams and families to support their health and well-being. Every day, you will monitor residents for changes in conditions, consult with the RN regarding resident conditions as needed, implement medication orders and treatments, and other duties as assigned. To be successful, you must be a dependable and compassionate LPN who communicates effectively, works well as part of a team, and is committed to enhancing the lives of older adults.

Wage Range: $30.00 - $32.00 / hour depending on experience

Position Type: Full-Time (40 hours per week), benefits eligible position

Available Shifts: Monday - Friday, hours could be 10:00am to 6:30pm or 11am to 7:30pm or similar (can be flexible). No weekends!!

Licensed Practical Nurse Responsibilities:

• Displays a courteous attitude and respect for all residents, families, and staff.

• Coordinates, oversees, and provides resident-related care.

• Coordinates care with residents’ primary care teams and families.

• Promotes accountability among ULPs by ensuring medications and resident cares are documented accurately, completely, and promptly.

• Oversees medications and/or treatments in accordance with physicians’ orders and monitors for effectiveness and response.

• Manages medications, supplies, and equipment, including ordering, receiving, storing, and disposing of items in accordance with policies and procedures.

• Documents care using computerized medication administration and charting software.

• Consults with the RN regarding resident conditions and changes in status.

• Notifies the Clinical Manager, attending physicians, and family members of significant changes in a resident’s condition and takes follow-up action as necessary.

• Monitors nursing personnel on duty during the assigned shift.

Licensed Practical Nurse Qualifications:

• MUST have 1 year of Assisted Living experience as a Nurse.

• Must be in good standing with the Minnesota Board of Nursing and have a current MN LPN license.

• Must have strong communication and organizational skills.

• Self-starter with a desire to be a part of a team.
