Are you a people person who loves helping others find their way?
New Horizon Counseling Center is looking for a compassionate, organized, and motivated Health Home Plus (HH+) Care Manager to join our team.
This is an opportunity to make a real difference in the lives of individuals living with serious mental illness, chronic health conditions, and complex social needs.
As an HH+ Care Manager, you won't spend your entire day behind a desk. You'll meet people where they are—in their homes, shelters, hospitals, and communities—and help connect them to the care and resources they need to move forward.

What You'll Do
You'll serve as a trusted partner to clients during some of the most important transitions in their lives.
Your day-to-day work may include:

Helping clients transition safely from hospitals, psychiatric facilities, detox programs, and rehabilitation settings back into the community.

Building relationships with clients through face-to-face outreach and community visits.

Developing individualized care plans based on each client's medical, behavioral health, and social needs.

Helping clients schedule and attend medical and behavioral health appointments.

Connecting clients to housing, benefits, transportation, food, treatment, and other community resources.

Helping clients navigate complicated healthcare and social service systems.

Staying connected with providers, hospitals, discharge planners, family/support systems, and community organizations.

Identifying barriers to care and finding practical solutions.

❤ Advocating for clients and helping them work toward greater stability, independence, and wellness.

Maintaining timely and accurate documentation and care coordination records.
Who We're Looking For
You don't have to have the exact title of "Care Manager" on your resume to be successful in this position.
We are interested in candidates with experience in:
- Case management, Behavioral health, Social work, Community outreach, Human services, Hospital or discharge planning, Housing services, Substance use services, Care coordination or Other direct-service positions working with vulnerable populations
Required:

Bachelor's degree in Social Work, Psychology, Human Services, Nursing, Public Health, or a related field.

At least 2 years of experience working with individuals with serious mental illness, co-occurring disorders, chronic health conditions, or other complex needs.

Strong communication, organization, and problem-solving skills.

Ability to build trusting relationships with clients and community partners.

Ability and willingness to work in the community and conduct regular face-to-face visits.
-
Reliable transportation and ability to travel to client and community locations.
Preferred:
-
Experience working with Medicaid, housing programs, benefits, behavioral health providers, hospitals, or community-based organizations.

Bilingual/multilingual skills.

Master's degree
