Physical Therapist (PT) Rate: $34 - $38.88/hour (depending on experience) Shift: Full-Time Schedule: Days

Chautauqua Nursing & Rehabilitation Center is looking to add a Physical Therapist (PT) to its rehabilitation team. In this role, you'll evaluate residents, develop individualized treatment plans, and provide skilled therapy services that help residents improve mobility, strength, balance, and overall function. RESPONSIBILITIES:

  • Evaluate residents and develop individualized physical therapy treatment plans

  • Establish short- and long-term rehabilitation goals and monitor resident progress

  • Provide skilled therapy services to improve mobility, strength, balance, endurance, and functional independence

  • Supervise Physical Therapist Assistants (PTAs) in accordance with New York State regulations

  • Complete required documentation, evaluations, care plans, and billing records accurately and timely

  • Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes

  • Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs

  • Recommend and coordinate durable medical equipment (DME) and discharge planning needs

  • Participate in care conferences, staff education, orientation, and quality improvement initiatives

  • Maintain therapy equipment, supplies, and treatment areas

  • Perform additional duties as assigned

REQUIREMENTS:

  • Master's or Doctorate Degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association

  • Valid New York State Physical Therapist License required

  • Newly licensed Physical Therapists encouraged to apply

  • Skilled nursing facility experience preferred

  • Strong clinical, documentation, communication, and problem-solving skills

  • Ability to work collaboratively within an interdisciplinary healthcare team

  • Commitment to providing compassionate, resident-centered care
