Position Overview
As anOccupational Therapist, you'll provide skilled occupational therapy services designed to help residents achieve their highest possible level of function and independence.
You'll evaluate residents, develop and implement individualized treatment plans, document progress, collaborate with physicians and other members of the care team, and contribute to care planning and discharge planning.
You'll work closely with residents, families, nurses, aides, therapy professionals, and other disciplines to ensure therapy goals are aligned with each resident's needs and overall plan of care.

Responsibilities
What You'll Do
-
Initiate and provide skilled occupational therapy treatments in accordance with physician orders
-
Maintain appropriate communication with physicians regarding resident therapy needs and progress
-
Complete comprehensive evaluations and maintain accurate clinical documentation
-
Maintain progress notes, MDS documentation, and discharge summaries for residents participating in occupational therapy
-
Develop, implement, and update individualized care plans
-
Collaborate with nursing, therapy, social services, and other healthcare professionals regarding resident needs
-
Participate in Care Planning meetings and provide clinical input regarding resident progress and goals
-
Maintain accurate treatment and statistical records
-
Communicate with residents and families regarding therapy goals, progress, and recommendations
-
Recommend and request supplies and equipment necessary to provide effective treatment
-
Participate in staff education and in-service training programs as requested
-
Support continuity of care during leadership absences as assigned
-
Provide supervision and direction to COTAs and Occupational Therapy Aides when applicable
-
Follow all applicable clinical, safety, compliance, and documentation standards
-
Perform other duties as assigned
Qualifications
Occupational Therapist:
-
Graduation from an approved school of occupational therapy required.
-
State license to practice occupational therapy required.
-
Two years’ experience in occupational therapy preferred.
-
Interest in occupational therapy treatment programs for the geriatric and subacute populations utilizing a team approach.
-
Ability to direct others necessary.
-
Ability to communicate well verbally and in writing required.
-
Current, valid CPR/BLS credential
-
Sense of responsibility and ability to work well with residents, families, and other staff members required.
-
Qualities of maturity, diplomacy, and ability to work well with others required.
-
Ability to work with geriatric and chronically ill residents, as well as other professionals in a friendly and helpful manner necessary.

Ability to perform tasks to established standards of excellence required.

This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
