Full job description
The Center at Blue Hills located in Stought, MA has an exciting opportunity available for a Full-time and PRN Physical Therapist (PT) to join their new in-house therapy team!
The Center at Blue Hills is proud to provide care for and specialize in Alzheimer's Care and Dementia Care.
63 Beds, Skilled Nursing Facility with primarily Psych and Dementia caseload.
$3,000 Sign on Bonus!!!
Key Responsibilities:
-
Perform initial physical therapy evaluations and create personalized treatment plans based on patient goals and needs.
-
Deliver physical therapy services to restore mobility, reduce pain, and improve physical function and independence.
-
Collaborate with an interdisciplinary care team including nurses, occupational therapists, speech therapists, and physicians.
-
Educate patients and families on home exercise programs, safety strategies, and wellness practices.
-
Supervise Physical Therapist Assistants (PTAs) and ensure adherence to treatment plans.
-
Maintain accurate and timely clinical documentation in accordance with state and federal regulations.
-
Participate in care planning, team meetings, and quality improvement initiatives as needed.
Qualifications:
-
Graduate of an accredited Physical Therapy program (DPT, MPT, or BSPT).
-
Current State Physical Therapist license in good standing (or eligible).
-
Current CPR certification (or willingness to obtain upon hire).
-
Strong clinical, organizational, and interpersonal skills.
-
Previous experience in a skilled nursing or long-term care setting is preferred but not required.
