About the Role
We are seeking a compassionate and experienced Hospice Nurse Practitioner (NP) to provide face-to-face patient visits and clinical oversight throughout our South Jersey service area. This role is essential in ensuring high-quality, patient-centered hospice care while supporting compliance with regulatory requirements and collaborating closely with the interdisciplinary team.
Key Responsibilities

Conduct face-to-face (F2F) hospice eligibility visits in accordance with Medicare guidelines

Perform comprehensive patient assessments to determine continued hospice eligibility
-
Provide oversight and support to hospice teams across Bergen, Passaic, Morris, Sussex, and Essex counties

Collaborate with physicians, RNs, social workers, and care teams to develop and update individualized care plans

Ensure accurate, timely, and compliant documentation in the EMR

Participate in interdisciplinary team (IDT) meetings as needed
-
Provide guidance and education to staff regarding hospice regulations and best practices

Assist in maintaining compliance with state, federal, and accreditation standards

Support quality improvement initiatives and patient care outcomes
Qualifications
-
Active and unrestricted New Jersey Nurse Practitioner license
-
Board certification as a Nurse Practitioner (FNP, AGNP, or related specialty)
-
Hospice and/or palliative care experience strongly preferred
-
Strong knowledge of Medicare hospice regulations, including face-to-face requirements
-
Excellent clinical assessment and critical thinking skills

Ability to work independently and manage a regional territory

Strong communication and interpersonal skills

Valid driver’s license and reliable transportation (travel required throughout North Jersey)
