Salary Range: $21.00 To $28.00 Hourly
Role and Responsibilities
The Client Relationship Specialist is a LPN who is responsible for developing and maintaining client relations and intake of direct patient referrals made to HCR .
Essential Functions

Cultivate relationships with our physicians and hospitals to facilitate the best Home Care solutions for their patients.

Monitor HCR RightFax folders, label faxes and attach electronic documents to patient record in Home Care Home Base (HCHB).

Monitor Intake Team email.

Produce timely, accurate data entry.

Responsible for entering orthopedic prescreens into HCHB according to agency protocol.

Provide phone support to the department and triage of calls to Intake staff.

Provide excellent customer service to internal and external customers.

Participate in department and agency meetings.

Other duties as assigned .

Education Requirements
- Graduate of an accredited practical nursing program and licensed to practice in NYS.
Qualifications and Requirements

Minimum of 4 years of experience in a clinical setting.

Exceptional customer services and communication skills required . Must be highly proficient and efficient working with a computer and be detail oriented and able to work in a fast-paced environment .
Work Environment
The Client Relationship Specialist is primarily in an office setting and may be exposed to outdoor conditions.
The working conditions are classified as sedentary work:
- Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
