Full job description

Rate: $18 USD per hour

Description

The role of the Day Habilitation Counselor teaches adults with developmental disabilities independent living skills, personal awareness, provides community access, and serves as a liaison with the community and Vanderheyden. Works with adults with developmental disabilities in community-based settings, assisting in all areas of daily living and communication. Full-Time day shift, 8 am-4 pm.

Primary Duties & Essential Functions: Implements and monitors each Day Habilitation Program with Walls.

  • Ensures valued outcomes outlined in the Individualized Service Plan (ISP), individual, and with treatment team input follows Residential Habilitation Plan.

  • Provides documentation regarding the implementation of each habilitation plan.

  • Obtains knowledge of the methodology of the plan to ensure correct implementation.

  • Evaluates feedback regarding habilitation plans in a timely manner.

  • Collaborates with the Coordinator to ensure that all individuals’ needs are addressed through the habilitation plan.

  • Provides full-time supervision and habilitative services in a standalone therapeutic program.

  • Responsible for providing pre-employment training at the local volunteer site and providing instruction in travel and/or community safety.

  • Other duties as assigned.

Required Education, Knowledge, and Skills:

  • Associate's degree preferred.

  • Must have at least 2 years’ experience in serving individuals with developmental disabilities.

Abilities and Working Conditions:

  • Must be available to work an AWAKE day shift.

  • Must have a valid NYS driver’s license.

  • Must be able to lift and carry up to 25 pounds with or without reasonable accommodation.

  • Willing to be flexible in terms of working autonomously in the community.

  • Must have intermediate skills in Microsoft Word and Excel.

  • Ability to deal positively with changing priorities in a fast-paced environment.

  • Exhibits a professional, courteous demeanor with internal and external constituents.

Pay: $18.00 an hour
