Responsibilities

Primary Care in the MCHS SEMN region is seeking a Nurse Practitioner or Physician Assistant to work as part of a multidisciplinary team to provide care to those living within our community through the Senior Service Skilled Nursing Facility. Within in this position the NP/PA will work collaboratively with primary care physicians to provide holistic care including acute, chronic, preventive and palliative care services to patients in nursing homes and skilled nursing facilities. The provision of care includes but is not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments and non-pharmacologic therapies, and counseling patients. Ideal candidates have experience in Palliative Care, Primary care and/or Geriatrics.

During the selection process you may participate in an OnDemand (pre-recorded) interview that you can complete at your convenience. You will have the opportunity to re-record your answer to each question – Mayo Clinic will only see the final recording. Qualifications

New Grads will not be considered at this time. 2+ years of APP experience.

Nurse Practitioner: Masters-prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current MN RN and APRN license and certification as a FNP, ANP or GNP.

Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the MN State Board of Medical Practice.

Additional Qualifications:

Active BLS certification from American Heart Association or American Red Cross at start date and DEA registration required
