Aftercare Treatment Specialist (CV2477-1) - Reentry Services Division

Aftercare Treatment Specialist

Support reintegration. Deliver care beyond supervision.

Volunteers of America Delaware Valley is seeking a dedicated Aftercare Treatment Specialist to support individuals as they transition from structured supervision to independent community living. This role plays a vital part in maintaining treatment continuity, ensuring post-release stability, and addressing the long-term needs of justice-involved individuals.

As a member of the treatment team, you’ll assess risks and needs, develop individualized service plans, provide counseling, and support clients through every phase of their aftercare journey.

What You’ll Do:

  • Conduct client assessments and create recovery-oriented service plans

  • Facilitate one-on-one case management sessions and group workshops

  • Monitor client progress, document outcomes, and revise treatment plans as needed

  • Provide linkage to community resources such as housing, employment, and mental health services

  • Attend court and parole hearings when required and maintain compliance with funder reporting standards

  • Facilitate core curriculum programming and assist with house meetings

  • Coordinate discharge planning and long-term support strategies

  • Participate in treatment team meetings and staff development trainings

  • Conduct drug and alcohol screenings and maintain confidential case documentation

  • Provide support to program operations and assist with data and reporting

What You Need:

  • Bachelor’s degree, preferably in a Human Services field

  • At least 2 years of professional experience in mental health counseling or working with special needs populations

  • Knowledge of addictive behaviors, group dynamics, and human behavior

  • Strong written and verbal communication skills

  • Basic computer skills

  • Core curriculum facilitation competency or willingness to certify

  • Valid driver’s license and ability to meet agency driving standards

  • Ability to work flexible hours, including some evenings and weekends

  • Must be able to pass a criminal background clearance

  • LSI-R training and certification (must be obtained within 90 days of hire)
