$5000.00 SIGN ON BONUS for Full Time Physical Therapist *** $2500.00 SIGN ON BONUS for Part Time Physical Therapist
Chase Memorial Nursing Home and Rehab is looking for a Physical Therapists to join our exceptional Therapy team!
Position Summary:
Full time- 30-40 hours per week- $5000.00 SIGN ON BONUS
Part-time - under 30 hours per week - $2500.00 SIGN ON BONUS
Per Diem positions available
The PT evaluates and treats residents who demonstrate disorders of musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits. Evaluation and treatment will only be conducted after a written order is received from the physician.

Essential Job Duties and Responsibilities:
The PT is responsible for the screening of residents of the facility as follows:

New facility residents – within 24 hours of admission

Long Term residents – upon receipt of a significant change form and/or referral from nursing

The PT is responsible for the completion of all necessary documentation, which includes but is not limited to the following items: initial evaluation forms, weekly progress notes, monthly recertification forms and discharge summary.
Essential Job Requirements
Education: Doctoral Degree in PT, Master's of Science Degree or Bachelor of Science Degree in Physical Therapy.
License: State Licensed Physical Therapist.
Qualifications: Must be 18 years of age. Ability to read, write and speak the English language
Experience: Preferred working in a long-term care facility.
