Monday - Friday 10pm - 6:30am
Position Summary:
A member of the Personal Care Home team who is responsible in assisting in the overall care of the residents including staffing. Resident care encompasses any needs of a resident that meets admission criteria regardless of race, religion, age, or cultural background. Majority of services provided for elderly. Must be 21 years of age.
Education, License & Cert:
LPN PA License
Experience:
Two years’ experience in geriatrics/long term care preferred, business experience helpful. LPN license in the Commonwealth of Pennsylvania. Graduate of accredited course for LPN. Current CPR certification. Ability to read, write, understand written and oral communication. Must have verbal ability for teaching residents, family members, other staff, and public.
Essential Functions:

Compliance with rules and regulations of all federal, state, and local governments is a requirement of employment.

Maintains PCH regulatory compliance.

Monitors and assesses residents’ medical issues.
a. Communicates with resident, responsible party, and physician to address issue.
b. Maintains accurate documentation.

Monitors medication administration to assure regulatory compliance.

Completes resident assessments within assigned timeframe.

Supervises direct care staff.
a. Orientation of new employees.
b. Assists with competencies and performance appraisals.
c. Provides education as needed.

Assists with admissions and discharge of residents.

Directs daily meetings “huddles” to assure all resident needs are discussed and addressed.

Keeps informed of fire, safety, and disaster programs.

Serves as designated representative in Administrators absence.
