The Role
The home health nurses will be responsible for providing high quality and coordinated care to patients in
their homes and communities. The ideal candidate will have a strong background in home health care, an
understanding of state and federal regulations, and a passion for helping patients live their best lives.
What You Will Do
- Conduct comprehensive patient assessments in the home setting to evaluate the patient’s physical and emotional health.
- Develop and implement individualized care plans based on physician orders, patient goals, and clinical best practices.
- Administer medications, injections, wound care, and other treatments as prescribed by the physician
- Provide patient and caregiver education on disease management, medication usage, home safety, and recovery techniques.
- Coordinate care with other healthcare providers, including physicians and therapists, to ensure continuity of care. Requirements
Requirements
- Current and unrestricted RN license in the state of Florida.
- Strong clinical skills and ability to perform evidenced based nursing practices.
- 1+ years of clinical experience in a healthcare setting, preferably in home health, hospice, or community health.
- Excellent communication and interpersonal skills with the ability to work eƯectively and independently with patients, families, and healthcare providers. Benefits
