Join UVM Health’s Adult Home Care team and help patients maintain independence, improve mobility, and enhance overall well-being—all in the comfort of their own homes. We are looking for therapists that are available weekdays, no weekends available at this tie. Must have prior PT experience; home health experience preferred but not required!
As ahome health physical therapist, you’ll be part of acollaborative interdisciplinary teamincluding RNs, LPNs, LNAs, Occupational Therapists, Speech-Language Pathologists, Social Workers, and administrative staff 🤝, providing comprehensive, patient-centered care.

About the Role
As anAdult Home Care Physical Therapist, you will:

Perform comprehensive evaluations and assessments 🩺

Develop individualized treatment plans tailored to each patient’s needs ✍️

Deliver skilled therapy interventions to improve mobility, strength, and function

Support patients in remaining in their homes safely and independently
Our coverage area is segmented into four territories withinChittenden and Grand Isle Counties. Per diem support all of the 4 territories.
Minimum Qualifications✅
-
Bachelor’s degree or higher inPhysical Therapy
-
2+ years of general PT experience, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
-
Home health experience preferred, but not required, we can train the right person!

Current Vermont Physical Therapist license, or willingness to obtain one
