Internal Medicine Provider
Salary: $155,000+/Year based on experience
LaSante Health Center is seeking an Internal Medicine Provider to join our multispecialty, community-based health center in Brooklyn. The ideal candidate will be committed to delivering comprehensive primary and preventive care to adult patients, managing chronic conditions, and promoting overall wellness in a culturally diverse community. Creole-speaking candidates are strongly preferred.
Responsibilities:

Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management

Diagnose and treat acute and chronic medical conditions
-
Collaborate with multidisciplinary teams to coordinate patient-centered care
-
Counsel patients on lifestyle modification, disease prevention, and medication management

Maintain accurate, timely documentation in the electronic health record (EHR)

Participate in quality improvement initiatives and team meetings
-
Complete additional tasks as directed by supervisor
Qualifications:

Medical degree (MD, DO, NP, or PA) from an accredited institution

Board certification or eligibility in Internal Medicine or Adult Primary Care
-
Current New York State license or eligibility for licensure

Experience in community health or outpatient primary care preferred

Excellent communication and interpersonal skills

Bilingual Creole/English strongly preferred
