STATUS:Non-exempt, Full Time
Hourly Rate:$20.00 to $22.00/hr
SUMMARY
Advisors triage calls which come to NY Connects to help the customer access and be aware of long-term support services and public benefit entitlements. There are typically no caseloads (some customers do need short-term follow-up support) and there are no afterhours or on call responsibilities.
The NY Connects team works with a hybrid remote work model—one week at home, one week at Lifespan’s main offices which are located in Brighton. Hybrid remote is dependent upon home technology compatibility with phone app and is considered only after staff have completed successful training period, which is typically 4-6 months from hire date.

DUTIES AND RESPONSIBILITIES

Provide unbiased information and guidance to older adults, persons with disabilities, caregivers, and allied professionals through the Lifespan NY Connects helpline.

Assess needs and help with referrals, completing applications, locating resources, etc.

Share information, in a professional, non-judgmental, and culturally appropriate manner, about programs and services that support an individual’s independence, choice, and self-determination.

Complete database documentation in an accurate, timely fashion.

Maintain a professional knowledge of services and meet core skill competencies. Participate in agency learning opportunities, cultural competency, and corporate compliance training. NY Connects Advisors are also required to complete CADER Information and Assistance and Options Counselling Certificate programs within timeframes designated by NYSOFA.

Other relevant duties as assigned by the supervisor.
Requirements
EDUCATION: Bachelor’s in Social Work or equivalent degree.
EXPERIENCE: Two years in human services or case management, with at least one year in Aging or Disability.
*Must have knowledge of public benefits and community-based long-term supports and services (e.g. Medicaid, SNAP, Social Security).

Salary/Hourly Rate
Multiple factors will be considered for salary/hourly determination including, years of relevant service, skills required, education, and internal equity.
