Overview
$20,000.00 New Hire Sign-On Bonus!
Shift: Monday - Friday | 8:00 AM - 5:00 PM (+ holiday, weekend, + on-call rotation)

Zone 2 - Altoona, Pleasant Hill, Ankeny, + Polk City area

Zone 3 - Downtown + 50317 area ( Weekend Package + Mid-shift available!)

Zone 4 - Urbandale, West Des Moines, + Windsor Heights area ( Weekend Package available!)
As a Home Health Registered Nurse, you deliver compassionate, high-quality nursing care directly in patients’ homes, using strong clinical judgment, assessment, and teaching skills to support better health outcomes. You collaborate with physicians and the care team to help develop individualized care plans and implement physician-directed treatment. Responsibilities
-
Provide skilled nursing care during home visits, completing ongoing assessments and supporting individualized plans of care in collaboration with the RN Case Manager and interdisciplinary team.

Perform comprehensive initial and ongoing patient assessments, identify changes in condition, and promptly communicate updates to ensure safe, timely interventions.
-
Deliver patient and caregiver education on disease management, medications, equipment use, safety, infection control, and emergency preparedness to support independence at home.

Implement physician-ordered care and assist with care planning, including measurable goals, updates, and discharge planning to promote timely , successful outcomes.

Coordinate closely with physicians and care team members, obtaining and documenting orders, relaying patient changes, and supporting seamless care coordination.

Monitor patients for risk of hospitalization or emergency department use and intervene proactively to reduce avoidable complications.

Maintain accurate , timely clinical documentation, including visit notes, verbal orders, time tracking, and OASIS assessments in compliance with regulatory standards.

Participate in interdisciplinary case conferences, quality improvement activities, peer review, and care coordination to ensure high-quality outcomes.

Supervise home health aides and LPN staff as appropriate, ensuring safe care delivery and proper documentation of supervision.

Maintain productivity standards, uphold professional communication, and participate in rotating evening, night, and weekend coverage to support 24/7 patient care.
Qualifications
Education:
- Graduate of State Board approved program for Registered Nurses and valid license as Registered Nurse in state where providing care required .
Experience:
-
One (1) year nursing experience caring for similar patient population as to be assigned required.

Two (2) years of home care experience preferred
-
Two (2) year of nursing experience preferred.
License(s)/Certification(s):
-
Valid RN license in the state(s) of practice, including eligibility under a NLC multistate license required .

Dual licensed positions must obtain and submit proof of dual licensure within 90 days of hire/transfer; proof of application is acceptable during this period.

Valid driver’s license required .

Valid Auto Insurance required .

IA Dependent Abuse Mandatory Reporter Training in state (s) where providing care required within 90 days of hire.

I L Dependent Abuse Mandatory Reporter Training in state (s) where providing care required within 90 days of hire.

IA Child Abuse Mandatory Reporter Training in state (s) where providing care required within 90 days of hire.

I L Child Abuse Mandatory Reporter Training in state (s) where providing care required within 90 days of hire.

Basic Life Support (BLS) certification required through the American Heart Association.
