Salary Range: $58,500.00 To $77,211.00 Annually

Position Summary:

We're looking for a committed, passionate and professional Healthcare Case Management Supervisor for our Jackson Heights location. The ideal candidate will be responsible for providing supervision to the healthcare case management staff to ensure quality care is provided to Apicha CHC's patients. Works closely with patients and the multidisciplinary care team at regular intervals for conducting, monitoring and evaluating of progress for patients to achieve positive health outcomes.

Job Responsibilities:

  • People management and supervisory - responsible for day-to-day patient care and quality services delivery to the patients under the healthcare case management.

  • Direct care - provides direct case management services to children, families, and adults to ensure the services they receive are appropriate for their specific needs.

  • Quality Assurance and Quality Improvement - perform quality assurance and quality improvement activities

  • Collaboration and outreach - facilitate on-going care team communication for betterment of patients including note taking, agenda, setting, etc.

Qualifications:

  • Bachelor’s degree in related field (i.e., health, nursing, mental health, child and family studies, public health, or other human services fields) with 4 years of qualifying experience required. Advanced degree in related field with 2 years of qualifying experience preferred. May consider bachelor’s degree in non-related field with 6 years of relevant work experience.

  • One (1) year of supervisory experience to paid staff strongly preferred.

  • Experience working with target populations strongly preferred. The target populations: individuals with Serious Mental Illness, Children and family, immigrant, developmental disabilities, serious substance use disorders (SUDs), chronic hepatitis, LGBT, high risk of or living with HIV/AIDS and/or Asians and Pacific Islanders, and/or immigrants

  • UAS-NY certification required within 1 month of hire.

  • Appreciation of and commitment to Apicha’s mission and values required.

  • Excellent written and verbal communication skills in English required.

  • Ability to work effectively in both a collaborative as well as individual setting

  • Ability to use and adapt to various applications (Outlook, Word, Excel, PowerPoint, eCW, eICARE, AZARA, HCS, Healthix, MAPP, PSYCKES, etc) required.

  • Excellent customer service skills to address in resolving patients and/or providers concerns.

  • Ability to work flexible hours including some evenings and weekends.

  • Ability to multitask and manage multiple projects in a stressful environment is strongly recommended.
