Full job description

LaSante Health Center is seeking a Social Care Network (SCN) Screener & Navigator to assist Medicaid members through the Social Care Network process. The ideal candidate is organized, compassionate, detail-oriented, and comfortable working with clients by phone and navigating online systems.

Responsibilities:

  • Contact clients and confirm demographic and household information

  • Obtain and document required consent

  • Verify Medicaid eligibility and coverage

  • Conduct Social Care Network screenings and eligibility assessments

  • Identify eligible social care services and assist with referrals

  • Create and manage cases within the SCN platform

  • Follow up with clients and document all outreach and case activity

  • Maintain accurate, timely, and compliant records

  • Coordinate with providers and other team members to ensure clients successfully receive services

Qualifications:

  • Strong communication and computer skills

  • Excellent attention to detail

  • The ability to manage multiple cases efficiently.

  • Prior experience with the NYS 1115 Medicaid Waiver, Social Care Networks, Unite Us, or similar care-management platforms
