Responsibilities $5,000 Sign-On Bonus Available

 

The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.

Qualifications Education

  • Bachelor’s Degree in a health-related field like biology, exercise science, or kinesiology Required and

  • Graduate of an accredited program from a CAPTE-accredited school Required

Work Experience

  • Previous experience recommended 

Licenses and Certifications

  • PT - Physical Therapist Upon Hire Required

  • Must be currently licensed in Physical Therapy in the state of practice. Required

  • CPR - Cardiac Pulmonary Resuscitation Upon Hire Required

  • DL NUMBER - Driver License, Valid and in State Must have a current driver's license and vehicle insurance, and access to a dependable vehicle Upon Hire Required
