NURSE PRACTIONER - PEDIATRIC/ ADULT/ FAMILY PRACTICE
The Nurse Practitioner (NP) provides well and acute/chronic care to patients in a primary care setting within the scope of their specialty. The NP is responsible for complete evaluation and documentation of the health status of his/her patients. The NP has a physician collaborator to support him/her as they provide this care. The NP is involved in the education of patients and families. The NP will practice to the full scope of his/her license to provide care to a diverse population.

PHYSICIAN ASSISTANT – PEDIATRIC/ADULT/FAMILY PRACTICE
The Physician Assistant (PA) for Primary Care, Adult and Pediatrics, provides both well-care and acute/chronic care to patients across all ages, from infancy through adulthood, in a primary care setting or age limits within the scope of their license. The PA is responsible for the comprehensive evaluation and documentation of the health status of patients, ensuring high-quality care throughout their lifespan. The PA has a supervising physician to support him/her as they provide care and collaborates with other healthcare professionals, including specialists, to deliver holistic and coordinated care. The PA educates patients and their families on health management, preventive care, and treatment plans. The PA will practice to the full scope of his/her license to provide care to a diverse population.
Requirements
Current license to practice in NYS. Must maintain credentialing and privileges as granted by the Board of Directors and the Health Center. Must maintain current DEA registration. Is not excluded to participate as a provider in the Medicaid and Medicare Programs.
