Per Diem Weekend Nurse Liaison

Named by The Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community-based, mission-driven hospice and palliative care organization seeks a Per Diem Weekend Nurse Liaison to join its expanding weekend nurse team. This is an on-site position located at Newton Wellesley Hospital .

Job Responsibilities :

The position of Per Diem Weekend Nurse Liaison is to provide information about hospice services to patients, families, and hospital-based staff, to conduct clinical assessments, coordinate hospital discharge plans, and maintain professional relationships within hospitals, with a focus on the assigned hospital(s).

Schedule and Weekly Hours:

Saturday and Sunday, flexible shifts.

Education, Skills and Experience :

  • Licensed by the Commonwealth of MA as a Registered Nurse.

  • At least two years experience preferred in community health, oncology, hospice, or palliative care.

  • Excellent communication and interpersonal skills

  • Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families

  • Highly skilled at providing patient assessments and evaluations.

  • Excellent problem-solving skills

  • Competency with accepted standards of hospice nursing practice.
