Description
The Family Therapy and Support Services Program provides treatment to youth with psychiatric, emotional, and/or behavioral challenges. The target population includes youth adjudicated as Juvenile Delinquent, persons in need of supervision, those at risk of out-of-home placement, and youth in a diversion program to avoid out-of-home placement.
The Caseworker Assistant provides both administrative and hands-on support to the Family Support program by assisting in the provision of case management services to children and families.
Job Responsibilities:

Provide direct casework services, including emergency intervention services, to clients and their families based on need assessment and treatment plan.

Responsible for supervision of family visits and transportation of children and families in the Family Support program.

Assist with the maintenance of client files (scanning documents into Agency database, completing reports, performing assessments, etc.).

Research and collaborate with community resources to determine eligibility and accessibility of services for clients preparing to transition out of Family support and/or into adulthood.

Participate as a member of treatment team for each assigned case – responsible for communicating with other team members and the Department of Social Services to ensure integrated service delivery, case continuity, and keep team informed of client progress.
