Responsibilities

  • Provide skilled in-home nursing care through comprehensive patient assessments and ongoing monitoring aligned with individualized care plans.

  • Deliver safe, high-quality interventions as directed by the RN Case Manager, supporting optimal patient outcomes in the home setting.

  • Educate patients, families, and caregivers on diagnosis, safety, and self-care strategies to promote independence and prevent complications.

  • Coordinate closely with RNs and interdisciplinary team members to ensure seamless communication and continuity of care.

  • Identify early signs of clinical decline and take proactive steps to reduce hospitalizations and avoid adverse events.

  • Maintain accurate , timely clinical documentation in compliance with regulatory, payer, and accreditation standards.

  • Participate in care conferences, quality improvement initiatives, and clinical record reviews to support excellence in care delivery.

  • Apply safe, effective use of equipment and infection control practices to protect patients and staff.

  • Manage workload independently, prioritize effectively, and maintain productivity standards in a dynamic home health environment.

  • Build positive, professional relationships with patients, families, and care teams while upholding clinical and ethical standards.

Qualifications

Education:

  • Graduate of State Board approved program for Licensed Practical Nurses required .

Experience:

  • One (1) year of nursing experience caring for similar patient population as to be assigned required .

  • Two (2) years of home care experience preferred.

  • Two (2) years of nursing experience preferred.

License(s)/Certification(s):

  • Valid driver’s license required .

  • Proof of Auto Insurance required .

  • Certified Basic Life Support required within 30 days of hire through the American Heart Association.

  • Valid LPN license in the state(s) of practice, including eligibility under an NLC multistate license required .

  • Dual licensed positions : Team members must obtain and submit proof of dual licensure within 90 days of hire / transfer . Proof of application is acceptable during this period .

  • IA Child Abuse Mandatory Reporter Training in state(s) where providing care  required .
