Licensed Practical Nurse (LPN) Rate: $31- $34.24/hour (depending on experience) Sign-on Bonus: Up to $3,000 Applicable Shift Differentials Apply: Up to $2.00 extra Shift: Full-Time, Part-Time Schedule: Days, 6:30 am- 7:30 pm, every other weekend | Nights 6:30 pm - 7:30 am, every other weekend

Roscoe Rehabilitation & Nursing Center is looking to add a Licensed Practical Nurse (LPN) to its nursing team. In this role, you'll provide high-quality nursing care, administer medications and treatments, and help ensure the safety, comfort, and well-being of residents. RESPONSIBILITIES:

  • Provide nursing care in accordance with resident assessments, care plans, and facility policies

  • Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice

  • Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties

  • Accurately document care, treatments, observations, and medication administration

  • Supervise and support Nursing Assistants to ensure quality resident care

  • Maintain up-to-date resident care information and communicate pertinent information during shift changes

  • Follow infection control, safety, resident rights, and confidentiality standards

  • Participate in interdisciplinary care planning and quality improvement initiatives

  • Assist with admissions, transfers, and other nursing responsibilities as assigned

REQUIREMENTS:

  • High School Diploma or equivalent required; Associate's Degree preferred

  • Valid NYS Licensed Practical Nurse (LPN) License required

  • Current CPR Certification required

  • Experience in a skilled nursing facility or with the geriatric population preferred

  • Strong clinical, communication, and organizational skills

  • Knowledge of medication administration, infection control, and resident care practices

  • Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team

  • Compassionate, resident-centered approach to care
