Occupational Therapist (OT) - Certified Home Health Agency (CHHA)

Status: Part-Time (20 Hours Per Week)

Salary: $52.27 per Hour

Assignment: Manhattan area

We are currently seeking a Part-Time Occupational Therapist (20 hours per week) to join our dedicated home care team.

Position Summary

The Occupational Therapist is responsible for providing and coordinating occupational therapy services for patients receiving care in the community. Working collaboratively with physicians, nurses, therapists, social workers, and other healthcare professionals, the Occupational Therapist develops and implements individualized treatment plans designed to improve patients' functional abilities, independence, and overall well-being.

Essential Duties and Responsibilities

  • Collaborate with physicians, patients, family members, nurses, social workers, and other members of the interdisciplinary care team to develop and implement individualized treatment plans.

  • Conduct comprehensive occupational therapy evaluations and ongoing patient assessments.

  • Develop, implement, and modify treatment plans based on patient needs, goals, and response to therapy.

  • Educate patients and caregivers on therapeutic techniques, adaptive equipment, safety recommendations, and strategies to maximize independence.

  • Monitor, document, and report patient progress, functional status, and treatment outcomes in accordance with agency guidelines and regulatory requirements.

  • Observe and report significant changes in patients' conditions to the physician and appropriate healthcare team members.

  • Complete OASIS assessments accurately and within established timeframes, in accordance with OASIS guidelines and regulations.

  • Complete all required clinical documentation, patient health status reports, and care records in a timely and accurate manner.

  • Supervise Occupational Therapy Assistants in accordance with New York State regulations and agency policies.

  • Coordinate care with other disciplines to ensure continuity of care and achievement of patient goals.

  • Participate in team meetings, case conferences, and interdisciplinary care planning activities.

  • Initiate and participate in case conferences for complex patient cases requiring multidisciplinary intervention.

  • Perform other duties as assigned.

Required Qualifications Education and Licensure

  • Current, unrestricted New York State license to practice as an Occupational Therapist.

  • Graduation from an accredited Occupational Therapy program approved by the American Occupational Therapy Association (AOTA) or another recognized accrediting body.

  • Master's Degree or Doctorate Degree in Occupational Therapy.

Experience

  • Minimum of one (1) year of occupational therapy experience preferred.

  • Home healthcare experience preferred.

Skills and Competencies Clinical Skills

  • Ability to perform comprehensive patient evaluations.

  • Ability to develop and implement individualized treatment plans for a variety of patient diagnoses and conditions.

  • Ability to establish measurable treatment goals and expected achievement dates.

  • Ability to assess patient progress and modify treatment plans as appropriate.

  • Strong clinical judgment and problem-solving skills.

Professional Skills

  • Excellent verbal and written communication skills.

  • Ability to educate and motivate patients and caregivers.
