Job Summary

EGA Associates is seeking a qualified Primary Care Physician to provide comprehensive primary care services at a Community Based Outpatient Clinic serving Veterans in and around Sherman, TX. The PCP will provide patient-centered care to assigned and unassigned patients presenting for services within an outpatient primary care environment. Working as a Primary Care Physician within the Patient Aligned Care Team, the PCP will assess and manage acute and chronic health conditions, develop clinically appropriate care plans, provide or arrange for needed services, and coordinate care with interdisciplinary team members. The PCP will practice in accordance with applicable licensure, certification, prescriptive authority, clinical privileges, scope of practice, and VA requirements.

Essential Functions

  • Provide primary care services to assigned and unassigned patients presenting for care.
  • Deliver care consistent with the physician’s licensure, clinical privileges, prescriptive authority, and approved scope of practice.
  • Assess, diagnose, treat, and manage acute and chronic medical conditions.
  • Ensure each patient’s care plan includes appropriate medical recommendations for clinically indicated care.
  • Offer clinically indicated healthcare services to patients assigned to the PACT.
  • Provide or arrange for care to which the patient consents.
  • Develop personal health plans in collaboration with patients and PACT staff.
  • Incorporate appropriate care management and care coordination into each patient’s treatment plan.
  • Order and interpret clinically appropriate diagnostic studies, laboratory testing, medications, consultations, and referrals.
  • Provide leadership to the PACT, including the appropriate delegation of care activities and processes to qualified team members.
  • Maintain accurate, timely, and complete medical documentation in accordance with VA and clinic requirements.
  • Perform additional duties as assigned.

Qualifications

  • Doctor of Medicine or Doctor of Osteopathic Medicine degree from an appropriately accredited institution.

  • Evidence of education, training, and professional experience in Internal Medicine or Family Medicine.

  • Board certification by an American Board of Medical Specialties-recognized board in Internal Medicine or Family Medicine, or by the Bureau of Osteopathic Specialists in Internal Medicine or Family Medicine.

  • Current, unrestricted physician license in state where CBOC is located

  • Ability to successfully complete all required VA and EGA credentialing, privileging, background screening, and onboarding requirements.

  • Current authorization for prescriptive authority when performing Primary Care Provider duties.

  • Current Drug Enforcement Administration registration when required by the assigned clinical duties.

  • Current Basic Life Support certification or ability to obtain certification before beginning patient care.

  • Knowledge of primary care practices, chronic disease management, preventive care, care coordination, and outpatient clinical operations.

  • Ability to provide care consistent with applicable VA clinical standards, policies, procedures, and documentation requirements.

  • Ability to work effectively within an interdisciplinary, team-based model of care.

  • Strong verbal and written communication skills.

  • Ability to use electronic health records, telehealth platforms, and standard computer applications.

  • Ability to read, analyze, and interpret clinical guidance, professional standards, technical procedures, and governmental or organizational requirements. Preferred

  • Previous experience providing primary care to Veterans.

  • Previous experience working in a VA medical center, VA outpatient clinic, or contracted VA Community Based Outpatient Clinic.

  • Previous experience serving as a Medical Director, Physician Director, Site Medical Director, or clinical team leader.

  • Familiarity with the VA Patient Aligned Care Team model.

  • Experience with quality assurance, peer review, utilization review, chart audits, and clinical performance improvement.

  • Experience providing or coordinating virtual and telehealth-based care.

Supervisory Responsibility

The Primary Care Physician provides clinical oversight and leadership to the clinic’s healthcare team. The position may provide functional clinical direction to advanced practice providers, nurses, and other PACT personnel. Direct employment supervision, disciplinary authority, and performance-management responsibility will depend on the final organizational and contractual structure.

Travel

Travel is expected to be negligible unless required for orientation, training, meetings, or other contract-related activities.

  • Maintain all required licenses, certifications, credentials, privileges, and prescribing authorities.

  • Complete required training, documentation, quality reviews, and administrative responsibilities within established timeframes.
  • Immediately report patient safety events, compliance concerns, clinical risks, and other matters requiring escalation.

Expected Hours of Work

Your work schedule may vary depending on the contract with the Client. Your start time should be discussed and approved by your facility supervisor if you wish to deviate from the normal operating hours. Employees may be required to work additional hours in order to successfully complete work obligations. Employees are required to obtain prior approval from their facility supervisor before working overtime.
