About the Role: Join Tradition Home Health Care as a Home Health Aide (HHA) and make a meaningful impact in the lives of our clients. We are seeking compassionate and dedicated individuals to provide essential support and care to those in need in Long Island, NY!

Responsibilities:

  • Provide personal care and assistance with daily living activities.

  • Administer medications and monitor client health as directed.

  • Assist with mobility and physical exercises to enhance client well-being.

  • Maintain a safe and clean environment for clients.

  • Document and report changes in client condition to the healthcare team.

  • Support clients with meal preparation and nutrition guidance.

  • Engage clients in social and recreational activities to promote mental health.

  • Communicate effectively with clients, families, and healthcare professionals. Requirements:

  • Current Home Health Aide certification in New York State.

  • Previous experience in home health care or a related field.

  • Ability to work flexible hours, including evenings and weekends.

  • Strong communication and interpersonal skills.

  • Compassionate demeanor with a commitment to client care.

  • Reliable transportation to travel within Nassau County.

  • Basic understanding of medical terminology and procedures.

  • Ability to lift and assist clients as needed.
