Full job description

Job Summary and Responsibilities

As our Social Worker you will proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team.

Every day, you will complete psychosocial assessments, interview patients, relatives, friends or significant others to obtain information about the patient’s personal, social and emotional history.

To be successful in this role, you must provides therapeutic care to patients to assist them in accomplishing treatment goals using interventions and techniques that are appropriate to the age and developmental level of the patient.

  • Coordinates treatment with community agencies such as social services, schools and probation.

  • Maintains social service records of each patient according to hospital and departmental program standards. Maintains statistics and turns in reports in a timely manner.

  • Communicates with crisis team for mental health emergencies. Coordinates outpatient treatment following inpatient hospitalization

  • Assists in revision of policies and provide input to the departmental budget by identifying departmental supply and equipment needs to meet departmental goals.

  • Adherence to Social Work Code of Ethics

Job Requirements

Required

  • Bachelor's Degree in Social Work

  • Licensed Clinical Social Worker: MN (LCSW:MN)

Preferred

  • Master's Degree in Social Work
