This role uses assessment data to determine appropriate service tracks and develops comprehensive strategies that promote self-sufficiency through coordinated support and workforce development. Conduct initial and ongoing assessments to determine participant needs, strengths, and barriers to employment. Assign participants to appropriate W-2 service tracks based on eligibility and assessment outcomes. Collaborate with families to establish employment and self-sufficiency goals, prioritizing barrier removal and skill development. Develop and implement wraparound strategies including job readiness, career planning, budgeting, and skills training. Coordinate activities outlined in the W-2 Individual Employment Plan (IEP) with those of external support providers. Monitor participant progress, update service plans, and adjust strategies as needed to ensure goal attainment. Maintain accurate and timely documentation in compliance with W-2 program standards.
