HomeWell Care Services is seeking a confident, relationship-focused professional with at least one year of relevant experience to expand our presence throughout North Jacksonville and Nassau County, including Callahan, Yulee, and Fernandina Beach.

This is not a position for someone who drops off brochures.

We are looking for someone who can confidently walk into hospitals, rehabilitation facilities, senior living communities, physician offices, and professional organizations, identify the right decision-makers, and begin developing genuine referral relationships.

You will represent a locally owned home care agency helping older adults and individuals maintain their independence, safety, and quality of life at home.

What You'll Be Responsible For

  • Develop relationships with hospitals, rehabilitation and skilled nursing facilities, senior living communities, home health and hospice organizations, physician practices, elder law professionals, veterans' organizations, and community partners

  • Follow targeted weekly outreach routes developed with agency leadership

  • Identify discharge planners, social workers, case managers, administrators, and referral decision makers

  • Conduct meaningful conversations to understand organizational needs and referral processes

  • Schedule follow-up meetings and introductions for agency leadership

  • Maintain consistent contact with high-value referral partners

  • Represent HomeWell at networking meetings, community events, health fairs, and professional gatherings

  • Document visits, contacts, conversations, and next steps in the agency's tracking system

  • Submit accurate activity documentation the same day

  • Immediately communicate qualified opportunities or families needing assistance

  • Assist with approved presentations and community outreach initiatives

  • Protect confidential information and follow HIPAA, compliance, and professional conduct requirements

What Success Looks Like

Successful performance includes consistent completion of outreach routes, meaningful conversations with referral decision makers, new qualified contacts, follow-up meetings with high-potential partners, accurate same-day documentation, and demonstrated movement of relationships from introductions to active opportunities.

We value the quality and progression of relationships—not simply the number of brochures distributed.

Who We're Looking For

You are confident, warm, professional, personable, and persistent. You are comfortable speaking with healthcare and business professionals and can independently manage routes, contacts, and follow-up commitments.

You should be motivated by measurable goals and personal accountability, communicate clearly in person, by phone, and in writing, and be comfortable using mobile applications and basic CRM/tracking systems.

Preferred Experience

At least one year of relevant experience is required, with two or more years strongly preferred, in one or more of the following:

  • Healthcare marketing or business development

  • Home care, home health, hospice, or senior living

  • Community outreach

  • Medical or pharmaceutical sales

  • Relationship management or outside sales

  • Veterans' services or senior service organizations

Position Requirements

  • High school diploma or equivalent

  • Valid Florida driver's license

  • Reliable, insured transportation and active automobile insurance

  • Ability to travel throughout the assigned territory

  • Smartphone with active service

  • Ability to pass required background screening

  • Availability during weekday business hours

  • Occasional availability for community and networking events

Compensation & Benefits

• $24.00–$28.00/hour, based on relevant experience

  • Approximately 15–20 hours per week

  • Mileage reimbursement for approved business travel

  • Flexible weekday scheduling

  • Performance-based bonus opportunities after the introductory period

  • Potential to move into a full-time role based on demonstrated results, including client conversions and revenue growth
