Social Worker - Clinical Review Team and Team Lead
Setting: Healthcare Facilities
Job Type: Contract - Full Time
Pay: $32 / Hour
Hours: 39.00
Location: Augusta, ME
The Social Worker - Clinical Review Team and Team Lead collaborates with the multi-disciplinary team to provide input in the development of the plan of care for those clients requiring social work intervention. The Social Worker - Clinical Review Team and Team Lead will provide support, advocacy, and resources to individuals and families facing various challenges. This role involves conducting assessments, developing care plans, coordinating services, and collaborating with interdisciplinary teams to promote overall well-being.
Minimum Requirements:

Bachelors or Master’s Degree in Social Work (MSW) from anaccredited institution required
-
Current state licensure as a Social Worker - Clinical Review Team and Team Lead (LSW)required or current state registration as a Registered Social Worker (RSW)depending on state in which they will be working.

Minimum of one year of social work experience preferred
-
Complies with all relevant professional standards ofpractice
-
Current CPR if applicable
-
TB questionnaire, PPD or chest x-ray if applicable

Current Health certificate (per contract or stateregulation)

Must meet all federal, state and local requirements

Must be at least 18 years of age
