Principal Objective/Role Overview:

This NYS-licensed Clinician provides individual evaluation services to effectively engage and serve children, youth and families/caregivers to determine and/or maintain their eligibility to receive the range of supports provided by Graham’s Health Home Program. The Clinician views each child/youth holistically and values the opportunity to meet the child/youth and their caregiver in their home, conducting a thorough, fully informed assessment of the strengths and needs of the client and their caregiver(s).

This individual is a New York State licensed practitioner of behavioral arts.

Job Responsibilities:

  • Conducts home or community-based face-to-face clinical evaluations and assessments of children and youth, including collateral sessions with caregivers, as well as reviewing all available clinical notes and evaluations (in accordance with the Children and Family Treatment Support Services – Other Licensed Professional requirements and mandates).

  • Establishes Medical Necessity eligibility for kids and youth needing additional services.

  • Conducts Complex Trauma assessments to establish Health Homes and other services eligibility.

Qualifications

  • Licensed practitioner of the behavioral arts (LMSW, LCSW, LMHC, LMFT) with a background in working with children & families

Salary & Compensation:

  • Base Salary or Hourly Rate: $52.00/hour
