Community Based Day Support Professional

Community Connections Inc. is in search of dedicated Community Based Day Support Professionals to join our Community Based Day Support team in South Yarmouth, MA. In this role you will support individuals with disabilities by creating a pathway to employment by providing career exploration & skill building activities. You will partner with individuals & businesses throughout Cape & Islands & surrounding communities & aid Community Connections in their mission to provide optimal independence & enriching services to people with disabilities. We are looking for someone with a commitment to compassion, mentoring others & serving the community. What You'll Do as a Community Based Day Support Professional: Develop & exp& opportunities for community integration to address the pre-employment, social, & recreational interests of the individuals we serve. Conduct assessments of the individuals? interests, skills & abilities to identify learning styles, social skills & available supports. Promote the development of skill acquisition & exponential learning through community exploration. Maintain relationships with community businesses & resources to secure opportunities for individuals we serve. Support individuals in becoming active, engaged members of their community by facilitating volunteer, leisure, & employment opportunities. Document individuals' progress toward goal objectives & keep up-to-date case records. What You'll Need as a Community Based Day Support Professional: High School Diploma or General Education Degree (GED) Bachelor's Degree in human service or business-related field preferred 1+ years' experience &/or training working with individuals with disabilities Valid driver's license, access to a motor vehicle, & satisfactory driving record Ability to work a flexible schedule Excellent written & verbal communication & interpersonal skills Demonstrated ability to work independently A proven ability to work independently & a proficiency in computer & word processing skills Who You Are as a Community Based Day Support Professional?
