Full job description
Care Coordinator – Home Care ❤️ Make a Difference Every Day! Do you enjoy helping people, keeping things organized, and being the person others can count on? Are you someone who finds joy in solving problems, supporting others, and making a difference behind the scenes? We are looking for a dependable, compassionate, and organized Care Coordinator to join our home care team! In this role, you will help connect the right caregivers with the clients who depend on them, keep daily operations running smoothly, and serve as a reliable point of contact for caregivers, clients, and families. Your work will help ensure that quality care continues, schedules stay on track, and people feel supported. What You'll Do * Keep Things Running Smoothly: Coordinate caregiver schedules, manage call-offs, and arrange coverage when changes arise. * Support Our Caregivers: Answer questions, communicate schedule updates, and help resolve everyday concerns. * Care for Our Clients: Assist with service coordination, client updates, and routine communication with clients and their families. * Be a Friendly Voice: Monitor and respond to incoming phone calls, texts, and emails with professionalism and kindness. * Solve Everyday Challenges: Take initiative to resolve routine scheduling and service issues while knowing when to ask for additional guidance. * Keep Us Organized: Maintain accurate client and caregiver files, electronic records, communication logs, and other documentation. * Stay on Top of the Details: Complete data entry, filing, follow-ups, and administrative tasks that keep our operation moving forward. * Follow Through: Make sure requests are addressed, information is communicated, and important tasks are completed. * Work as a Team: Collaborate with the Director/Senior Care Coordinator and bring complex concerns or decisions requiring management approval to their attention. What Makes You a Great Fit? * You are organized, dependable, and take pride in doing things well. * You enjoy helping people and communicating with kindness and professionalism. * You can juggle multiple priorities without losing sight of the details. * You are a problem-solver who looks for solutions rather than waiting to be told what to do. * You take ownership of your responsibilities and follow through on commitments. * You are comfortable using computers and learning new systems. * You understand that sometimes the smallest details make the biggest difference. What We're Looking For We want someone who brings a positive attitude, a caring heart, and a willingness to learn. You don't have to know everything on day one, but you should be ready to grow, take initiative, and become a trusted part of our daily operations. You will work under the direction of our Director/Senior Care Coordinator, handling routine operational needs independently while seeking guidance on more complex situations. Why This Role Matters Home care is about people. Behind every schedule is a client counting on us, a caregiver showing up to make a difference, and a family trusting us to help care for someone they love. As our Care Coordinator, you will play an important role in bringing all those pieces together. Your organization, communication, and follow-through will help make a meaningful difference every single day. If you love helping people, enjoy keeping things organized, and want your work to have a purpose, we'd love to hear from you!
