Registered Nurse- Health and Wellness Coordinator

We are searching for a Health and Wellness Coordinator for our small personal care home serving older ladies and gentlemen, most with a diagnosis of ID. Can be flexible with hours but in general they would be Mon-Fri 9a-5:30p. Pay is on a monthly basis. Duties include but are not limited to: 1. Oversee medication administration program as well as pass medication during scheduled shifts 2. Oversee overall health for older population of ladies and gentlemen with a diagnosis of ID/ DD 3. Complete indicated assessments (i.e. monthly, weekly, urgent etc) 4. Schedule and complete all indicated physician, specialist visits/ diagnostic as indicated by individual need. Both on/off site and virtual. Company vehicles available. 5. Noting of completed appointments ensuring indicated follow up is implemented. Qualifications: 1. Current PA Nursing license (RN) 2. Experience with elder care, psych and/ or ID 3. Ability to work independently and as part of a team 4. Strong organizational skills 5. Desire to have both hands on care as well as administrative duties 6. Dependable and passionate about helping others 7.
