Medical Records & Transportation Coordinator

Elevate Care is seeking a detail-oriented and organized Medical Records & Transportation Coordinator to join our team. This position is responsible for managing and maintaining resident medical records while coordinating transportation arrangements for medical appointments, hospital visits, and other off-site services. The ideal candidate is organized, dependable, and able to manage multiple responsibilities while maintaining accuracy, confidentiality, and regulatory compliance.

KEY RESPONSIBILITIES

Medical Records Management

• Maintain and organize resident medical records in compliance with HIPAA, Medicare/Medicaid, and applicable state and federal regulations.

• Ensure accurate and timely documentation, filing, scanning, and retrieval of medical records.

• Monitor records for accuracy, completeness, and compliance.

• Conduct routine chart audits and follow up on missing or incomplete documentation.

• Respond to medical records requests from authorized personnel while maintaining confidentiality.

• Assist with admissions, discharges, transfers, and record retention procedures.

• Collaborate with nursing, clinical, and administrative departments to ensure proper documentation.

• Maintain electronic health records and update resident information as needed.

Transportation Coordination

• Organize, schedule, and coordinate resident transportation for medical appointments, hospital visits, and other off-site services.

• Arrange transportation through facility vehicles, outside transportation providers, and other appropriate services.

• Coordinate appointment dates, pickup times, return transportation, and scheduling requirements.

• Communicate transportation arrangements with residents, families, nursing staff, and outside providers.

• Maintain an organized transportation calendar and ensure appointments and transportation needs are addressed in a timely manner.

• Verify transportation arrangements and assist with resolving scheduling changes or transportation issues.

• Coordinate necessary paperwork and documentation related to resident appointments and transportation.

QUALIFICATIONS

• Previous experience in medical records, healthcare administration, or a skilled nursing facility preferred.

• Knowledge of HIPAA regulations and medical records procedures.

• Experience coordinating appointments, scheduling, or transportation services preferred.

• Proficiency in electronic health record (EHR) systems and Microsoft Office.

• Excellent organizational, communication, and time-management skills.

• Strong attention to detail and ability to manage multiple priorities.

• Ability to maintain confidentiality and work collaboratively with residents, families, and facility staff.
