THE POSITION

• Location: Lakeville, MN (55044)

• Hours:

• Days Friday through Sunday 12-hour shifts

• Overnights Friday through Sunday 12-hour shifts

• Client Specifics: 4-year old with trach and a g-tube

• Location: Lakeville, MN (55044)

• Hours:

• Overnights Friday, Saturday & Sunday 12-hour shifts

• Client Specifics: 18-year old with trach, vent and a g-tube

• Location: Lakeville, MN (55044)

• Hours:

• Days Saturday 16-hour shift

• Days Sunday 12-hour shift

• Overnights PRN/Casual 12-hour shifts

• Client Specifics: 9-years old a g-tube*

TRAINING & SUPPORT

Join our dynamic team and get ready for some serious teamwork! We offer specialized, hands-on training in ventilator, trach, and g-tube care—so no experience is required to get started. We’ll help you breeze through the learning curve and into a role that truly makes a difference. With 24/7 support from our dedicated team, you’ll never feel alone. Pediatric experience is a plus, but if you’ve got heart and a willingness to learn, you’ll fit right in with our warm and welcoming crew!

Night & Weekend Bonus Structure - You choose the shift. We add the boost.

• Weekday Nights (Mon–Fri): +$1/hour

• Weekend Nights (Sat–Sun): +$2/hour

• Weekend Days (Sat–Sun): +$1/hour

SALARY RANGE

At PHS, we are committed to providing specialized care for medically complex children. Every role, from direct patient care to customer support, plays a vital part in improving the lives of kids and families. The impact you make is invaluable. We are dedicated to offering fair and transparent compensation to our team members. The base range for this role is $25.00 to $31.00 hourly for LPN and $35.00 to $41.00 hourly for RN, which is based on license, relevant experience, and qualifications.
