Great care starts with a great plan, and the person behind it.

At HomeWell Care Services, we believe exceptional care depends on thoughtful planning, clear communication, and strong relationships. As our Care Manager, you will be the trusted link between our clients, their families, our caregivers, and the healthcare community in South Orange County.

As a locally owned husband-and-wife team, we take pride in high standards of care, clear communication, and dependable operations. This is a key leadership role in a growing agency. You will help shape how we assess, plan, and deliver care, and you will have a direct voice in building a team and culture we’re proud of.

We are looking for an experienced, highly responsive Care Manager to lead client assessments, develop personalized care plans, and make sure every client receives consistent, high-quality care. You will be the first point of contact for families and referral partners. When a family calls or a client’s needs change, you act quickly and keep everyone informed.

SERVICE AREA & SCHEDULING We serve clients throughout South Orange County, with a focus on Newport Beach and Laguna Beach, and surrounding communities including Irvine, Aliso Viejo, Laguna Niguel, Dana Point, San Juan Capistrano, and San Clemente. This is a full-time in-person position, primarily on weekdays, with regular in-home visits across our service area. The role includes participation in client assessments, supervisory visits, referral & community partner visits, and on-call rotation for urgent client and scheduling needs.

RESPONSIBILITIES Lead the delivery of compassionate, well-coordinated care. Responsibilities include:

  • Conducting in-home client assessments and reassessments, and developing individualized care plans

  • Matching clients with caregivers based on needs, preferences, skills, and compatibility

  • Serving as the primary contact for clients and families, responding promptly to questions, concerns, and changes in care needs

  • Performing supervisory visits and quality checks to make sure care plans are followed

  • Building and maintaining relationships with referral sources such as hospitals, skilled nursing and rehab facilities, assisted living communities, physicians, case managers, and senior care professionals

  • Representing HomeWell at community events and networking opportunities

  • Supporting caregiver onboarding, training, and ongoing coaching

  • Maintaining accurate client records and documentation in line with California Home Care Organization requirements

  • Participating in an on-call rotation for urgent client and scheduling needs

QUALIFICATIONS

  • 3+ years of experience in home care, home health, hospice, assisted living, or a related senior care setting, including client-facing care coordination or management

  • Demonstrated experience conducting client assessments and developing care plans

  • Established professional relationships with referral sources in Orange County, ideally in the Irvine / Newport Beach / Laguna Beach areas

  • Exceptional responsiveness and communication skills with clients, families, caregivers, and professional partners

  • Strong organizational skills and comfort managing multiple priorities in a fast-paced environment

  • Background in nursing (RN, LVN), social work, gerontology, or a related field preferred

  • Reliable transportation, a valid California driver’s license, and auto insurance

  • Proficiency with scheduling and care management software, email, and mobile communication

  • Ability to perform the essential duties of the position, with or without reasonable accommodation

  • Ability to meet applicable background screening requirements

WHAT YOU CAN EXPECT FROM US

  • Competitive pay: $70,000–$85,000 per year, based on experience, plus referral bonus

  • Real impact: A leadership role in a growing, locally owned agency where your ideas are valued

  • Respect and support: Owners who are accessible, listen, and back you up

  • High standards: A commitment to professional operations, clear expectations, and quality care

  • Prompt application responses: Clear next steps and guidance to help you move through the hiring process

  • Coverage: Employer-provided liability insurance and workers’ compensation coverage

  • Additional benefits: Mileage reimbursement at the IRS rate, monthly phone stipend, on-call stipend, referral bonus, paid time off and holidays, and professional development support
