Most liaison roles hand you a territory and tell you to go build relationships. This one hands you 12–15 qualified inbound leads a month, pays you on every hour your clients receive for as long as they stay on service, and staffs 100% of the referrals you accept — first shift within 48 hours.
WHY THIS IS DIFFERENT
You get leads, not just a map. Our paid search and SEO produce 12–15 qualified inquiries a month, handed to you. Current close rate is 25%. Beating it is worth money to you.
Your commission doesn't expire. Uncapped, paid per hour served, for the lifetime of the client — not a one-time fee, not 90 days and done. A client you close in March is still paying you in year three. Higher rate for clients you source yourself, lower for company leads.
We staff what you sell. 100% fill rate, 48 hours to first shift. You know what it costs to sell a discharge planner on a Friday case and have your agency fail to cover it. Press us on this number in the interview.
You report to the owner directly. No regional manager, no approval chain — decisions on cases and rates happen same-day.
WHAT YOU'LL DO
Own the referral network — hospital and SNF discharge planners, case managers, elder law attorneys, senior living directors, physician practices. These become your relationships, and retention is yours too.
Convert the inbound. Every lead routes to you with a 15-minute response standard. You run the in-home consultation and close private-pay and LTC insurance business yourself.
Build the payer side. Secure UnitedHealthcare and Horizon BCBSNJ MLTSS approvals. Deepen relationships with MLTSS care managers at Aetna and Fidelis, where we're already contracted. Pursue Medicare Advantage supplemental benefit contracts under SSBCI and VBID.
WHAT YOU BRING
- 2+ years selling home care, hospice, home health, or SNF admissions in New Jersey
- Referral sources you can name by organisation and role — people who take your call today
- A track record in numbers: clients closed, hours added, close rate
- Experience being paid on commission, and fluency in how your own plan works
- Ability to close an in-home consultation with an adult child deciding under pressure
- Valid driver's license and reliable transportation
MEASURED ON: new clients per month, net new hours per week, inbound close rate against the 25% baseline, response time, 90-day retention, MCO contracts secured.
COMPENSATION
- Base $50,000–$65,000, set by experience and verified referral network
- Uncapped commission per hour served, for the lifetime of the client
- 3-6 Months earnings protection up to $80,000 via declining monthly guarantee
- Realistic OTE $110,000+, no ceiling
- Separate bounty and quarterly milestones for Medicaid/MLTSS development
- IRS-rate mileage, company phone and laptop, marketing collateral
- Monthly business development budget; sponsorships approved separately
- 10 days PTO plus NJ Earned Sick Leave (1 hour per 30 worked, capped at 40/year)
TERRITORY
Private pay and LTC: Chatham, Livingston, Scotch Plains, Short Hills, Springfield, Summit, Warren, Westfield — across Union, Essex, Morris, and Somerset counties. Eight towns, tightly clustered. Four referral sources in a morning without spending it on the Parkway.
Medicaid/MLTSS: statewide, concentrated in Middlesex, Essex, and Union.
