Administrative Assistant / Care Specialist for Seniors

Location: Smithtown, NY

Hours: Approximately 30–35 hours per week (flexible scheduling)

Job Summary:

We are seeking a compassionate and detail-oriented Administrative Assistant / Care Specialist to join our team, supporting seniors in their homes and ensuring high-quality, person-centered care. This hybrid role involves both administrative responsibilities and client-focused care coordination. This position will also play a key role in handling inbound lead calls and supporting service conversions.

Key Responsibilities:

  • Take incoming lead calls and inquiries, professionally present service offerings, and support the conversion of leads into active clients.

  • Conduct initial home care assessments in client residences.

  • Explain service policies, including rates, clearly to clients and their families.

  • Develop individualized Care Plans based on comprehensive assessments and client input.

  • Complete all initial intake paperwork and manage the client registration process.

  • Collaborate with the staffing team to coordinate service delivery and care plan implementation.

  • Regularly reassess client needs and update care plans accordingly in collaboration with the Client Service Manager.

  • Perform general office duties including answering phones, communicating with clients/prospects, and supporting new program implementations.

  • Document progress notes and daily work activities using web-based platforms and tools.

  • Navigate technology platforms using smartphones, tablets, and other office equipment.

Qualifications:

  • Minimum 1 year experience in home care, elder care, or a related clinical setting.

  • Previous office and customer service experience required.

  • Experience handling lead intake and conversion is highly desirable.

  • Strong assessment skills preferred.

  • Excellent oral and written communication abilities.

  • High level of computer proficiency and comfort with technology.

  • Ability to work independently and collaboratively in a team environment.
