BE THE PERSON HOSPITALS AND CARE TEAMS TRUST TO GET PATIENTS SAFELY HOME

Nursing & Rehab At Home is looking for a relationship-builder to connect our agency with the hospitals, post-acute facilities, and care teams that send us patients. You’ll work side by side with discharge planners, case managers, physicians, and nurses to make sure patients leave the hospital with the right home care plan in place, and you’ll be the face of our agency to the people who depend on us.

Nursing & Rehab At Home is a clinician-owned and operated, family-run agency, proudly rooted in our local community.

Many of our employees have been working with us for over a decade! We offer a relaxed, nurturing environment where patients and staff are both well cared for.

WHY THIS ROLE

• Meaningful work: Your coordination helps patients transition home safely and sooner, with the right support waiting for them.

• Get out of the office: You’ll spend your time meeting people and building partnerships, not sitting behind a desk.

• A voice in strategy: You’ll hear firsthand what referral partners and patients need, and your insights go straight to leadership to help shape new and improved services.

• Collaborative team: You’ll work closely with our Sales Development Manager and intake team.

• Growth potential: Build market knowledge and relationships that open doors in sales, business development, and healthcare leadership.

• Real relationships. You'll get to know your patients and their families, and you'll see them get stronger, more confident, and more independent.

• A team that has your back. You're out in the community, but you're never on your own. With regular huddles, peer support, and celebrations, working with Nursing & Rehab At Home gives you colleagues who feel like family.

• Variety. No two days look alike. Different neighborhoods, different stories, different challenges.

WHAT YOU'LL DO

  • Partner with discharge planners and case managers at post-acute facilities to coordinate safe, timely discharges.

  • Consult with physicians, nurses, social workers, and other disciplines to build a coordinated home plan of care.

  • Start the referral process, gathering intake details, hospital course, face-to-face documentation, and plan of care, and help the intake team follow up on incomplete referral packets.

  • Be a trusted resource for referral partners, explaining our services, coverage, and how we can support their patients.

  • Build and maintain strong relationships with current and potential referral and payer sources.

  • Join patient care conferences and in-services.

  • Keep our reputation strong as a high-quality provider in the community.

  • Track the local market, including major payers, key referral sources, and competitors, and share what you learn with leadership.

  • Connect patients and families with community resources when we’re not the right fit.

  • Practice ethical, compliant outreach in line with federal, state, and local regulations.

WHAT YOU BRING

  • Bachelor’s degree in Marketing, Business Administration, or a related field (Master’s a plus)

  • 3+ years of healthcare experience, ideally in home health

  • 1+ year of sales experience preferred

  • Knowledge of sales and marketing strategies

  • Excellent communication, negotiation, and public relations skills

  • A confident, tactful approach to building relationships

  • Organization, flexibility, and the independence to manage your own time in the field

GOOD TO KNOW

This is a field-based role with regular travel to hospitals and facilities, including driving in varied weather. You may occasionally be in settings with infection risks, and we provide the training and precautions to handle them.
