Job Summary:

Leads coordination, partnership development, and systems-based problem solving at the intersection of Emergency Department (ED) utilization/post hospitalization and community-based care. Serves as a central patient care liaison across healthcare, social services, and public sector partners to improve care transitions, reduce avoidable ED use, and strengthen community-based care pathways. This position is a member of the Mobile Integrated Health team.

Education:

  • Bachelor’s Degree required

  • Nationally Registered Paramedic Certification or nursing degree required.

Experience:

  • 5+ years’ experience in the medical profession, which must include experience as an EMT, AEMT, Paramedic or RN.
